Volunteer Release Waiver 2024

PARTICIPANT GUIDELINES

• Complete the CPF volunteer registration and waiver online before participating. If you have already RSVPed via GivePulse for your clean-up event, you automatically completed the release waiver there. Anyone who has not registered via GivePulse MUST complete this release waiver before volunteering (for example, day-of walk-ups).

• Come prepared. Wear long pants, closed-toe shoes, sun protection, bug repellent, etc.

• All participants must wear protective gloves during cleanup. Gloves will be provided to you on-site. Beware of hazards such as sharp objects and glass – do not pick up these items. Do not reach into leaves or bushes to pull out trash. If you come across hazardous debris that requires equipment you do not have, notify the cleanup leader, who will pass this information back to the Chicago Parks Foundation.

PARTICIPANT WAIVER

I acknowledge that I am solely responsible for determining if I/my minor child are physically fit and/or skilled for the activities and programs contemplated by this agreement. I further acknowledge that I have carefully read this entire form and I am aware that in signing below, I allow me/my minor child to participate in Chicago Park District and Chicago Parks Foundation programs and activities. I expressly assume the risk and legal liability for participation in this regard. I waive and release all claims for injuries, damages or loss which I or my minor child may sustain as a result of participating in any and all such activities.

I recognize and acknowledge the possibility of physical injury associated with Chicago Park District and Chicago Parks Foundation activities and programs, and I voluntarily agree to assume the full risk of any injuries, damages or loss, regardless of severity, that my minor child or I may sustain as a result of said participation. I further agree to relinquish all claims and fully release and forever discharge any claims that I or my child may have (or accrue to me or my minor child) as arising out of, connected with, or in any way associated with participating in said activities and programs as against the Chicago Park District or Chicago Parks Foundation and/or its, officials, officers, employees, volunteers or agents.

I agree to defend, hold harmless and indemnify the Chicago Park District, Chicago Parks Foundation, its commissioners, officials, officers, employees, volunteers or agents as to any claim that I or my minor child or any other representative on my child's behalf may seek to assert relative to my minor child's participation in Chicago Park District and Chicago Parks Foundation activities and programs.

I hereby grant permission to the Chicago Park District or Chicago Parks Foundation for the use of any and all photos and videos in which I or my child(ren) may appear. The usage is inclusive of, but not limited to, the publication or inclusion in brochures, posters, catalogs, websites, handbooks, banners, and broadcast or print advertisements. I agree to waive any claim to compensation for use of said photos. 

I hereby grant permission to the Chicago Parks Foundation and Chicago Park District the use of my email to be imported into their mailing list to receive any and all forms of communication via email.

I have read and fully understand this entire form warning of risk, assumption of risk, waiver and release of claim and indemnification.
